Do You Need a Massage Therapy Facility (Establishment) License?

FACILITY (ESTABLISHMENT) LICENSURE: Massage establishments must now be inspected before the Board will issue an establishment license. Please be advised that the inspection of a facility will not be performed if the facility is under construction and/or if all the appropriate city/town permits required for the operation of the facility have not been obtained. Failure to ensure that the facility is ready for inspections will result in a delay in processing the application for licensure. If you wish to submit an application for a single therapist or multiple therapist massage establishment, please go to the Applications and Forms page to download the application. Please be advised that upon receiving your completed application and all required documents, the Board will review your application to ensure that you meet all the requirements for licensure prior to forwarding your application to the Office of Investigations to schedule an inspection. A Board investigator will then contact you to arrange an inspection. Please note that establishment licenses are required for facilities that offer massage therapy, unless exempted from licensure per the Board's regulations. Operation without a license may incur fines of up to $1,000 for a first violation of the regulations.
